Specimen Collection Supplies Request
Unit representatives responsible for ordering specimen collection supplies should contact the Repository Ordering Officer via commercial/DSN line to discuss orders. For all collection supply orders, an AFRSSIR Supply Order Request must be completed by the unit and faxed or emailed to the Repository Ordering Officer.
- Orders should be placed 30 days in advance. Order only supplies needed, no excess stock.
- Allow four - seven days for ground delivery.
- Rush orders 24 - 48 hours allowed for deploying units.
- All venipuncture supplies, including gloves, are the unit's responsibility.
- P.O. Box addresses will not be processed.
- Unit representative should sign ordering form prior to submission.
- Print a copy of the supply order form, fill it out, sign it and fax or email it back.
Specimen Destruction Requests
Per DOD Instruction 5154.30, Section 3.c.(1), dated Dec. 29, 2015, individual specimen samples will be destroyed at the request of the donor following the conclusion by the donor of completed military service or other applicable relationship to the DOD. To be eligible, a service member must have completed their military service obligation verified by the Defense Eligibility Enrollment and Records System and DD Form 214.
A request must be made in writing and include:
- Service members name,
- Social security number,
- Date of birth,
- Branch of service or civilian status (if applicable), and
- Mailing address.
All requests must include a copy of the service member's DD Form 214 verifying separation and end of service obligation.
Specimen Database Queries
The roster can contain any information, but at a minimum must contain the SSN and names of the personnel. It can be submitted in any one of the following computer file formats:
- Microsoft Excel (all versions through Excel 2000)
- Microsoft Access (all versions through Access 2000)
- Delimited text or Fixed-width text

About Rejected Specimens
Beginning in June 1998, the Armed Forces Repository of Specimen Samples for the Identification of Remains implemented a rejected specimens program to assist units in the field in identifying service members whose DNA reference specimens were not accessioned into the Repository.
Typical reasons for a specimen to not be entered into the Repository include, but not limited to:
- Missing, incomplete or incorrect Social Security Number
- Insufficient or no blood on specimen card
- Filter paper containing blood separated from specimen card
- Excessive fungus growth due to incomplete drying at collection
When a specimen is rejected by the Repository, the unit where the specimen was collected will receive written notification, which consists of a letter and list of rejected specimens by service member name, SSN and reason for rejection. The specimen(s) will have to be recollected by the unit. The rejected specimen(s) previously sent to the Repository are destroyed and not returned to the collection unit.
The Repository also receives duplicate cards which are bloodstain cards of service members that are already in the system and only one card per service member can be accessioned. All duplicate cards are destroyed. Installations can eliminate the number of duplicate cards they submit by implementing the Verification Process.
