About the High Reliability Maturity Index-Model

The DHA uses Ready Reliable Care’s High Reliability Maturity Index-Model to measure progress toward high reliability. The model assesses high reliability capabilities to show where organizations are strong, where gaps exist, and where focused improvement will have the greatest impact. It’s not a performance rating.

The model evaluates capability maturity across the four domains of change:

  • Leadership Commitment
  • Culture of Safety
  • Continuous Process Improvement
  • Patient Centeredness

In 2025, the model focused on inpatient facilities. Beginning in 2026, it is expanding to include outpatient and dental clinics. 

Maturity is scored across four phases—Beginning, Developing, Advancing, and Approaching—to help leaders understand where their organization is in its journey.
