Our goal is to be the primary treatment option for all TRICARE-eligible beneficiaries—service members from all branches, retirees, and family members—and veterans—from all locations across the Military Health System. Active duty service members may be eligible for funding for travel expenses if referred from outside of the catchment area of our locations.
Maxillofacial Restoration is required only for the most complex patients in need of dental rehabilitation coupled with microvascular reconstructive surgery. Unlike traditional reconstruction, which may focus solely on surgical repair, our program emphasizes complete restoration. The Maxillofacial Restoration Program treats:
- Active duty service members from all branches
- TRICARE-eligible beneficiaries
- Veterans

Maxillofacial Restoration Center of Excellence leaders worked with the DHA Integrated Referral Management and Appointing Center to create clinical criteria for cases to be prioritized for virtual consultation by or referral to a Center of Excellence location for treatment.
MTF providers may initiate a referral for virtual consultation at the Center of Excellence using MHS GENESIS Referral Management 2.0.
Managed care support contractors will work with TRICARE Health Plan and the DHA Integrated Referral Management and Appointing Center to assess beneficiaries’ eligibility to receive care at this Center of Excellence based on target clinical referral criteria.
Care at the Maxillofacial Restoration Center of Excellence locations is delivered through a coordinated, multidisciplinary team tailored to the patient’s individual care needs. Our team-based approach supports continuity of care from initial evaluation through recovery and rehabilitation. Services may include:

Maxillofacial Restoration is a low-volume, high-acuity process required only for the most complex patients in need of dental rehabilitation coupled with microvascular reconstructive surgery. Maxillofacial Restoration involves multidisciplinary coordination across nine fully-integrated clinical care pathways spanning from perioperative care to rehabilitation.
The Maxillofacial Restoration Center of Excellence sites focus on comprehensive care for patients with the following conditions of the face, mouth, and jaw:
- benign pathology
- malignant pathology
- congenital anomalies
- primary (acute) trauma
- secondary (delayed) trauma
- secondary dental restoration following previous free flap reconstruction
Providers across a spectrum of related clinical disciplines may identify patients in need of Maxillofacial Restoration care. Please contact the Maxillofacial Restoration care team via the Clearinghouse or directly to discuss a potential referral.
